If any information (name, address, social security number or EIN) has changed from the dates noted below or if you do not have a W-9 form on file with McMurray Art League, please send a newly signed form to us. The IRS requires us to issue a 1099-MISC form to each instructor who has earned $600 or more from us within a calendar year.
